Enrollment Procedures
At the time of registration you will receive the following:
- Child Registration Form
- Child Information and Development Form
- Health Appraisal Form
- Medical Authorization Form
- Medical Emergency Care
- Parent Handbook
Before the child's enrollment you will be required
to complete the Child Registration Form and pay the first week's
tuition. The Health Appraisal form MUST be completed and returned
to us BEFORE the child's first day of school. This is a state
law and will be good for two years.
